What is Hospice Care?

Hospice is specialized healthcare for those facing a life-limiting illness or terminal diagnosis. Care focuses on the patient’s comfort and quality of life so they can live each day as fully as possible.

Hospice care can be provided in private residences or wherever the patient calls home, such as skilled nursing facilities and nursing homes.

At Halcyon Hospice & Palliative Care, we support an Open Access model of care. Open Access gives patients the medical comfort and social support traditionally available through hospice care. At the same time, patients can continue to access medical treatments that may slow their disease.

Halcyon Hospice & Palliative Care is guided by our commitment to patient-centered care. We bring a clinically-directed, interdisciplinary team to nurture, support and heal during the last stages of an illness and into the bereavement period.

What you Need to Know about Hospice Care at Halcyon

Our primary focus is on you, the patient. That means:

  • we are available 24 hours a day, 7 days a week
  • we allow you to control your care and keep your primary care physician
  • all patients without regard to their ability to pay
